First column: Root, the bottom note of the chord

Second column: Dominant, play this with your left hand, remember to leave out the 5th note, the 3rd note in the chord spelling

Third column: + plus sign, it means you play it along with one of the 6 choices presented in the 6 columns on the same row

Six – Nine column: play the triad in the column. remember you can also use inversions as well
